My dad is at Encore and I couldn't have asked for a more wonderful home for him. The staff is caring and loving. The facility is kept spotless. The nurses Maureen, Magie and Roxanne are attentive, responsive and fun. The entire staff is wonderful, I couldn't imagine better caregivers for my father. The activities are engaging and fun for the residents. My brother and I feel blessed that our dad is at Encore, Crystal Lake!

The director of nursing needs a class in professionalism and common courtesy. The director of nursing did not know who I was nor was the one who initially left me a message. The director of nursing is not above the managing director or executive director, but she thinks she is the way she talks to customers. Furthermore, if the way the director of nursing treats others is any reflection of how new customers are treated when inquiring about the facility, or returning phone calls then run far away from Encore of Crystal Lake. If she treats a complete stranger like trash and with utter disrespect then I’d hate to know how individuals are treated within the facility as associates and family loved ones are treated as residents. The director of nursing needs to be fired after completing a class in phone etiquette 101 - identity yourself and your position. Vile personality. I’ve had working experience in over five AL/ALZ facilities; private home health, and she is terrible. Run far away.

I would like to express my appreciation to Executive Director Lyric. My wife Gail is a resident of the Crystal Lake facility. I was rather nervous and perplexed as I moved Gail from Clarendale Assisted Living to Encore. Lyric was very professional and put me at ease. She explained everything in detail and asked for my input. After talking with her, she put me at ease. Gail has been at Encore a month. I have met Lily, Maureen, Maggi and Shannon. All of her staff on the scale of 1 to 10, with 10 being the highest, I would rate them all 10. All staff are pleasant and caring. I feel I made a good choice sending Gail to Encore. Please take note you very lucky to have such good professionals. Good team members are hard to come by. I was in the work force for 50 years, so I speak from experience.

Encore and Staff are AMAZING! Jennifer is a joy to work with, and is compassionate and very informative. I suggest anyone looking for an all inclusive Memory Care facility to take a tour or speak with Jennifer to learn about the many amenities Encore offers to their lucky residents.

My Father was a resident at Encore Memory Care in Crystal Lake. The staff was always caring and attentive not only to my Father, but also my Mother and our whole family when we came to visit. I would recommend this facility as a dignified place to trust with the care of your loved ones.

Encore Memory Care did an outstanding job taking care of our father (Jesse Avila) during his last days. He had been to two other memory care units and was not having a nice time. Lyric, and several of the staff here made him feel at home and special. He finally passed but he was well taken care of final days. I recommend Encore. 👍

My mother, Vivian, was at Encore for a short period of time before she passed away. We knew from the first visit that this was the place for our mom. During her stay, she was treated as family. They attended to her every need and made her feel special. They reminded her daily that she was loved. The Encore staff also treated our family as one of their own. When my mom was in the hospital, Lisa would visit her and bring candy and flowers. I would highly recommend this facility to anyone who is looking for a family environment for their loved ones.

My Father has lived at Revere Court for 6 years and I can’t say enough good things about that decision. The staff is amazing and attentive. The programs they offer to our loved ones are exciting and enriching on a daily basis. Placing my Father in a memory care home was the most stressful and difficult decisions I’ve ever had to make but I’m so thankful I chose Revere Court. They put all my worries at ease and have handled the pandemic gracefully and carefully to keep our family members safe and loved.

It is no small thing to highly recommend Revere Court of Crystal Lake as a wonderful place for loved ones with cognitive disorders to live as I, naturally, have very high expectations for the people that take care of my beloved mother. I moved my mom here from another facility because the level of care there was unacceptable and the management inadequate to make the necessary changes. Revere Court has neither of those problems. It was Revere Court’s management, in all departments, that convinced me to give them a try. Before moving in, I witnessed management never hesitate to come out of their offices to assist the caregivers and interact with the residents in a way that demonstrated they really do care about them. That serves as an example to the rest of the staff that the residents are not just work, but real people who just need extra love and attention. After moving in, management took the time with me to listen carefully and put together a detailed care plan that I, as the daughter and POA, was satisfied with. In the weeks that followed, it took time for the caregivers to learn what my mom needed but with the help of their supervisors, they learned what to do and they now do it well. Naturally, challenges do come up that need to be addressed but with caring and experienced management who are always available to listen, those challenges are addressed promptly. Revere Court also has very talented people who plan and execute all the fun activities that take place every day to keep the residents engaged and active. The housekeeping, maintenance and kitchen staff are excellent as well. Revere Court’s facility is clean and comfortable and feels like you’re sitting in someone’s cozy living room.

I don't know if there is a more difficult decision to make than turning over the care of your loved one to somebody else. The self-reproach for not being able to care for them yourself, wondering who will care for them like you would, how will they be treated, apprehension over surrendering them to an institutional existence . . so many doubts and concerns present themselves when placing a loved one outside the home.
Mom & dad both developed dementia at the same time. It began about 10 years ago. It caused a major disruption to their lives and to the rest of the family's as well. Devastating. Something as simple as trying to get them to take their medications would result in a contentious 20 minute ordeal. They often refused to eat. Toileting was another ominous challenge for us. It was really putting a strain on the family. We tried in-home assistance services, assisted living facilities and other memory care facilities. Their needs just weren't being met.
Then we heard about Autumn Leaves and their approach to memory care. That was a little over five years ago. We liked the fact that family members could visit at any time of the day or night without restriction. That felt very transparent to us. All of the staff, including the young people who assist the nurses and CNA's, are patient and kind with the residents. Residents are always treated with dignity and respect. In five years we have never witnessed a staff member become impatient with, or mistreat, a resident.
As for the food, we were glad to see that the meals are appetizing and nutritious. If they get hungry between meals, snacks are also provided. Somehow the staff always gets mom & dad to eat their meals and take their meds without incident.
We were relieved to see how clean the facility is. No lingering odors as we experienced in other facilities. The décor is updated regularly and the building and grounds are very well maintained. It has a comfortable feel to it, kind of like you're sitting in your own living room.
It was heartening to see mom and dad get involved and participate in the games and activities that are part of the program there. The stimulation seems to help. It also gives us peace of mind to know that there is a nurse on site 24/7 and a physician who visits weekly. It's convenient to have a beauty shop on the premises so there is no need to go through the hassle of transporting them to the hairdresser or barber. Really lightened the load on us.
As time passed, we were concerned that as mom & dad's disease progressed and their condition declined that Autumn Leaves might not be able to accommodate them. Looking back over the last five years, we realize that as mom & dad's needs changed, A/L's care plan adapted to meet those needs. Seemed to be nothing they couldn't handle.
The people at Autumn Leaves are what makes the difference. The entire staff is kind and committed to the residents. They genuinely seem to enjoy the interaction with the residents and take pride in the care they provide to each of them.
It's a real relief to know mom & dad are in the best possible place for them and they are being well cared for.

I cannot say enough wonderful things about Autumn Leaves of Crystal Lake. My mother has resided at the memory care facility for the past three years and I am grateful everyday for their exceptional care. The staff is very caring and attentive to every resident, assisting whenever needed. Activities are provided on a regular basis. I feel welcome whenever I visit my mom. As my mother’s dementia progresses, the Autumn Leaves staff and the nurses are always informing me of what needs to be done to continue her treatment. Their communication with family is wonderful, helpful and always knowledgeable. The facility is beautiful and makes residents and their family feel like they are home.
