My experience with LifeChoice has truly been a blessing. Having lost my mother in January and having to take on the role as full time caregiver for my father who is in stage 5 Parkinson’s disease, it’s been a challenging year to put it lightly. I didn’t know the first thing about caring for someone at the end of their life. I brought LifeChoice on in April and the team has been amazing. From the first meeting with his nurse, Grace, I knew I picked the right place. Her bedside manner and professionalism is amazing. Anytime I need a medication refill for my dad or supplies she’s immediately on it and I usually get them the same day. One of the things I appreciate about LifeChoice is the care for the caregivers. Elif, the social worker has been by our side throughout this journey. She’s done a great job setting up the respite care stays so my family and I can get a break. She’s also been a great resource in recommending caregivers to help me out as well. Last but not least, my dad’s CNA Diana, has been truly amazing. Her bedside manner is second to none. Every day that she comes my dad lights up. Diana truly cares about each of her patients. I could not recommend this team enough I am truly grateful for them..

We mainly worked with my father’s nurse and social worker. We were so lucky to have Grace Santiago on my father’s journey. She answered all my questions, even if I asked the same questions, she was so patient. I can't say enough positive things about the social worker Elif Kucuk at LifeChoice Hospice, she was our guide through this process. She’s an amazing resource and support. They went over and beyond the care for my father and helped us through a tough time. Thank you Grace and Elif!

I cannot express strongly enough how grateful my family and I are for the support and care my father received while in hospice care. The nurse Sahyan Perez was wonderful, she always took her time to explain everything. The social worker Elif Kucuk is an incredible person, she is not only professional at her job but she is humble, caring, compassionate and always listened to my needs or concerns. Paulina the CNA always kept my father comfortable. I cannot thank all of you enough. We are forever grateful!

Highly recommend the team from Life choice! Nurse Veronica took very good care at home for the last 3 days of my father's life. Everybody involved was very nice, answered all our questions. Couldn't ask for a better help! Thank you!

Every single staff member that my family encountered with LifeChoice was phenomenal. Being a member of the healthcare community, I have encountered many hospice programs, LifeChoice is by far the most professional and compassionate program I have ever worked with. LifeChoice provided superb, well-rounded care and support to my mother and family in the two months in 2024 when we were fortunate to be with this hospice. The nurses were always accessible and responsive to our changing needs, ensuring our questions were answered fully and letting us know gently and consistently what was normal to expect. My mother lived her last months in the best way possible, with much thanks to LifeChoice hospice services.

Everyone at Life Choice was wonderful. As my father declined they stepped up even more to make sure that he (and I) were both comfortable. Big shout out to social worker Sara, nurse Grace and CNA Claudia. The whole team made a very difficult time easier. Highly recommended.

If someone will ever need the most kind, caring, knowledgeable and 24/7 accessible hospice care for their terminal loved ones, this service is unsurpassed. Incredible support network for me as a care giver to my mother in her last stage of her journey.

Our time spent with hospice was short but the attention to detail, the genuine show of compassion and 24 hr availability was key to the care my husband received. We called LIFECHOICE Hospice after my Mom who had frontal lobe dementia and was rapidly declining. We sat down with Sara and immediately felt at ease and knew we would go with them. Marina was my Moms nurse and to say she was amazing would be an understatement. My Mom only needed her services for about 2 weeks as she passed away on October 9th. I would recommend this company to anyone who unfortunately would need hospice care

The people are wonderful but the organization leaves much to be desired. We had to call the answering service to relay a message to the team and the service was rude and said, "sorry we are very busy!" Then, when my father passed away we called the answering service as instructed by the care team and the service never relayed the message! We sat with my father's lifeless body for more than 4 hours! And now, nearly a week later, we are still waiting for someone to pickup the bed he died in, the commode, the oxygen tanks and tray table. At such a difficult time for the family, you would think that there would be more concern for prompt and caring responses. However, the team of caregivers including the social worker, nurses, chaplain and nurse assistants were very kind and comforting and we really appreciated them.

LifeChoice was the second hospice company we spoke with about our mother, Gena, and we're so glad we chose them. The CNAs were very gentle and caring, Alex helped us through the decision-making, and our nurse, Marina, was just wonderful. We called her often and got good advice to help us through this sad time. LifeChoice took care of all the DME, medicine, and other needs quickly and efficiently.

Kindness, compassion and 24/7 availability made a painful experience easier to get through. Thank you LifeChoice.

On behalf of my family we can’t say enough how much we appreciated all the kindness, care and compassion we received from Life Choice. My mother suffered from dementia and was eventually bedridden until she passed on 16th Jun. Marina was our hospice nurse. She is absolutely AMAZING. We consider her like family. She always went above and beyond for our family and our beloved mother. I know we could of not gotten through this without all her love and support she provided not just for my mom but she was a HUGE support to my dad. She also had an amazing team. Jessica was so awesome with my mother. She came twice a week to bath her. She showed so much compassion for my mom and also my dad. She also played an important part of keeping my mother comfortable. I was a volunteer here and Sarah, Lupe, Marinya and Veronica are wonderful people and really care about their patients. Highly recommend this place.

We had such a wonder experience with Betty, the CNA assigned to my mother. She is such a kind soul and was so helpful to my mom and my family. My mom truly bonded with Betty and created such a special relationship with her, my family and I. I can’t believe we only had Betty with my mom only 3 weeks but those 3 weeks were some of most special we have ever experienced. Pastor Doug was also so wonderful. I highly recommend requesting both!
